A HOUSEHOLD SHALL BE CONSIDERED TO BE ELIGIBLE FOR LIHEAP SUBSIDY WHEN THE HOUSEHOLD MEETS THE BASIC ELIGIBILITY CRITERIA

Box 367 WEST LIBERTY, KY 41472 www.gcscap.org “An Equal Opportunity Employer M/F/D/V” Low income home energy assistance Program (LIHEAP) to Help with Low Income Families with Their Heating Expenses Begins November 4, 2018 Gateway Community Action Agency will begin its Subsidy component of the LIHEAP program November 5th. The subsidy component’s official business will run from NOVEMBER 5TH THROUGH DECEMBER 14TH. Pre-applications for households on a fixed income will begin on October 16th - October 31st A household shall be considered to be eligible for LIHEAP Subsidy when the household meets the basic eligibility criteria: • Completes an application • Be responsible for home energy costs, directly or as an undesignated portion of their rent • Have a combined household income, for the calendar month prior to the month of application, not in excess of 130% of the federal poverty level as listed in the Income Eligibility Guidelines • Live in Kentucky when the application is made and apply in the county of residence, providing proof of your 911 address: and • Not have liquid resources in excess of $2,000 or $3,000 if at least one person in the household is 60 years old or disabled. ($4,000 if a member of a household has an illness which requires liquid resources to be accessed regularly for medical or living expenses) • Other required documentation includes: Proof of gross income from the previous month, proof of each members social security number, a copy of the most recent electric bill, most recent heating bill or verification from your landlord that the heating expenses are included in your rent Applicants should call their local outreach office to schedule their appointment for pre-applications. Applicants may begin calling their local outreach office on October 22nd to schedule a LIHEAP appointment. LIHEAP is a statewide initiative sponsored by Community Action Kentucky in partnership with the Cabinet for Health and Family Services (CHFS) Applications are taken in order of the last name of the head of the household. Choose the date on the calendar that contains the first letter of your last name. Applications will be taken from November 5, 2018 through December 14, 2018 PLEASE BRING THE FOLLOWING INFORMATION ON YOUR DESIGNATED DAY: 1. Proof of Social Security Number or Permanent Residence card (Green Card) for each member of the household 2. Proof of all household’s (all members) income from the preceding month. 3. Most current heating bill, statement from your landlord if heating expenses are included in your rent, statement from utility company if you participate in Pre-Pay Electric Program. 4. The account number and name on the account for main heating fuel source and electric bill
